Getting Started
15‑Minute Introduction – Complimentary ($0)
This is a video meeting where we’ll discuss your goals and interests, answer your questions about Design Psychiatry’s practice model, and review some of the potential advantages and disadvantages of seeking care through Design Psychiatry compared with other alternative paths towards your goals. If we both believe that establishing a partnership with Design Psychiatry seems to be the right approach for you, then we will review the next steps to best prepare us for working together, including setting a time for a next meeting and discussing how to prepare so that we can make the most of our time together.
No clinical services are provided during this preliminary conference, and no physician-patient relationship is established. A physician-patient relationship is established with mutual agreement at the conclusion of a “Comprehensive Evaluation and Treatment Planning” session, which is described below.

Comprehensive Evaluation & Treatment Planning
This is the first formal visit for a client seeking to establish as a patient with Design Psychiatry, and is typically a one hour visit scheduled during the 15 minute introduction meeting.
To prepare for this visit, clients complete a detailed self-inventory of recent functioning across different domains, medical history, psychiatric history, childhood/developmental history, family history, social history, and lifestyle factors. Prior to the visit, your self-assessment will be carefully reviewed along with any additional materials you would want your psychiatrist to be aware of from the beginning of your partnership. Design Psychiatry is committed to comprehensive and personalized care. If you write it, your psychiatrist will read it. This is your opportunity to really take the time to be thorough in telling your story. The more details you include about your background, goals, and challenges, the more refined and personalized the visit can become, with more time to focus in on coming up with a treatment plan together.
During this visit, you should expect to discuss how your experiences may be described within a “bio-psycho-social” model of mental health. A note regarding the use of mental health diagnoses in this model: There are significant and often underrecognized risks of harm that arise from pressures within the modern mental health industry to oversimplify, overgeneralize, and overpathologize common complaints and symptoms with the use of diagnoses defined with relatively loose and subjective rules of categorization. Although Design Psychiatry utilizes traditional (DSM-V/ICD-10) diagnoses to contextualize treatment decisions within an evidence-based, systems-oriented approach, Design Psychiatry emphasizes moving beyond nosological classification of illnesses and towards cultivating a flexible, strength-focused and person-centered perspective for each client. You should leave each visit with an understanding and agreement concerning any current working diagnoses that are being assigned to you in visit notes, applied to a superbill to be submitted to your insurance, or used to order any follow up labs/medications/referrals.
By the end of this visit, you should come to an agreement with an initial strategy to move forward towards your treatment goals. If your treatment goals are still unclear, this may include assignments around further clarifying your short-term and long-term goals or further defining anticipated challenges. You will discuss options to follow up with Design Psychiatry for medical services, therapy, a combination of these, and/or a referral to outside services as fits your situation.
Follow‑Up Medical Services
Follow up medical management visits are typically 20-30 minutes depending on complexity, though longer visit lengths are available for more difficult or in-depth diagnostic clarification, longer discussion opportunities, extended treatment planning, etc. These services may include ordering or review of labs, imaging, medications, outside records, discussion of other interventions/modalities/referrals, education, coordination of care, and lifestyle coaching around various environmental factors that can impact biological, social and psychological components of health.
Psychotherapy Only Follow‑Up
These visits are typically 30, 45 or 60 minutes for therapy or coaching without medical monitoring or intervention (including medication management), though longer visit lengths are available for some situations. They are billed at a lower rate (see Pricing Summary table in top right of this page (PC) or below (mobile)) to reflect the lower reimbursement that most insurers provide for this service.
Dr. Hawkins has received formal, supervised training and has extensive practice in Psychodynamic, Cognitive Behavioral (CBT), Dialectical Behavioral (DBT), Solutions-Focused, Positive Psychology, Interpersonal, and Family Systems therapy modalities. He additionally has had training with evidence-based treatment options utilized by the Veteran’s Administration for PTSD including Cognitive Processing Therapy, EMDR and Prolonged Exposure therapy, though he does not specialize in these forms of therapy. Most therapy Dr. Hawkins offers is Integrative Therapy, blending across multiple different theories and techniques from the above modalities into a single, cohesive framework tailored to a client’s specific goals, challenges, and preferences.
Integrated Medical & Psychotherapy Visits
These visits for combined medical services and therapy in a single visit are typically 45 or 60 minutes and are billed at the lower therapy rate after 45 minutes (see Pricing Summary table in top right of this page (PC) or below (mobile)).
Healthcare Advisory Services
Design Psychiatry offers several consultation services other than direct patient care which are available to the general public as well as to other licensed healthcare professionals and organizations. These include:
- Assistance understanding and navigating the mental healthcare system
- Research synthesis
- Educational or topic review
- Business or program development
- AI or digital tool development
These services are billed based on time (currently $200 per hour). Payment via credit card is required in advance. Specifics of service agreements are discussed during a complementary introductory visit. To schedule an introduction, please fill out this inquiry form with your desired contact info. Note: Unlike direct patient care services, these services do not have geographical restrictions for video conferencing services. These consultation services do not establish a patient-physician relationship, and patient-specific medical advice cannot be offered during this type of consultation. This form of general/educational consulting is not a substitute for medical care, and no emergency services are offered.
Pricing Summary

Note: Although all payments are cash-only via credit/debit card on file at time of service, a “Superbill” is generated for all clinical services which may be submitted to insurance for out-of-network reimbursement (which in many cases may completely cover the above service costs).
Locations & Populations served
Clinical psychiatry services are currently limited to Ohio, Michigan & Florida, though advisory consultation services (without any components of direct patient care) are not geographically limited. In-person appointments are offered in Medina, OH.
Design Psychiatry treats adults (18+) only. Please see FAQ for additional information about populations served.
Telehealth vs In‑Person Care (Pricing & availability differences)
Most appointment openings offered by Design Psychiatry are via secure telehealth vs in-person visits. Design Psychiatry does not maintain a private office building or dedicated auxiliary staff and instead operates from a combination of home offices and shared workspaces, allowing for more flexibility in physician scheduling and lower costs for telehealth services, which in turn allows for more savings passed on to clients.
Many clients prefer telehealth services because these visits are much less disruptive to their schedules, there are fewer mobility/geographical/transportation barriers, family/friends/supports can join and participate more often, greater privacy is possible, and/or they offer greater physical or emotional comfort and safety.
However, Design Psychiatry is committed to only using telehealth to provide care when it is a safe and effective alternative to in-person care. Physical examination beyond what is possible via telehealth is clinically necessary for some situations, such as for evaluation of certain neurologic symptoms or medication side effects. In other situations, a client may have a strong preference for an in-person visit for a more personal connection. When an in‑person visit is preferred by the client, or the psychiatrist believes physical examination is medically necessary, patients may choose to:
- Schedule an in‑person appointment with Design Psychiatry in a shared workspace (located in Medina, OH) with a facility surcharge ($200 per visit regardless of visit length) which reflects the added cost of room rental and physician travel,

- Be referred to another provider for physical evaluation with coordination of care with Design Psychiatry,
-or-
- Be referred for a transfer of care to another psychiatric provider
Commitment to Transparency
Transparency in all services and pricing is a core value of Design Psychiatry. This includes commitment to:
Advance notice of any service/fee changes. Any change in service fees for existing clients will be announced at least 90 days in advance via this website and through direct communication (email).
No surprise billing. All service fees are discussed up front to ensure the shared understanding that is critical for informed consent and for your ability to decide whether these services are worth your investment.
Care Between Visits
Most patient-physician communication should occur at scheduled video or in-person follow-up visits. Live, two way, face-to-face visits are essential for safe, thorough, and quality communication and treatment. Adequately budgeting time, space, and dedicated focus in client and physician schedules is essential to consistently delivering excellent care to everyone served by Deisgn Psychiatry.
However, there are times when a short conversation between visits makes sense for simple questions, or when “life happens,” and clients may need to bridge communication between scheduled visits. Physician availability for this type of communication is more limited though, and pricing for these services is higher to reflect this as well as to incentivize clients to prioritize communication during scheduled visits.
Telephone, Online Digital Medical Services, or other communication/paperwork completed outside of a regularly scheduled visit:
- Administrative communication (scheduling, pharmacy or lab questions, etc.) – not billed
- 1–10 minutes – $60
- 11–20 minutes – $100
- 21+ minutes – $200
As described above, these services do not substitute for video or in‑person appointments when a more thorough evaluation is clinically indicated (at provider discretion). You will always be notified when such billing applies. Detailed fee ranges for between‑visit services are also outlined in the Financial Policy & Fee Schedule, provided to all clients at intake.
Appointment Policies
Design Psychiatry treats both physician and client time as valuable. Adequately budgeting time, space, and dedicated focus in client and physician schedules is essential to consistently delivering excellent care to everyone served by Design Psychiatry. Protecting scheduled appointment times and planning ahead whenever possible when adjustments need to be made is considered a shared responsibility with associated costs for both physician and client when this responsibility is not met.
Client Cancellations
- More than 48 hours: no charge.
- Less than 48 hours: 50% of visit fee.
- Less than 24 hours: 90% of visit fee.
- No‑Call / No‑Show: 100% of visit fee.
If a client is missing at the time of an appointment, the physician will attempt to contact the patient to:
- Notify them of the missed appointment
- Reschedule or discuss continuation of care
- Assess for any urgent needs
If additional physician time is required outside the scheduled visit time, it may be billed as between‑visit care as detailed above.
Client Late Arrivals
- If a visit is shortened due to patient lateness, the full scheduled appointment time length still fee applies.
- If the physician offers (depending on availability) and the client agrees, the visit may be extended to allow for the originally scheduled duration and billed according to the fee schedule.
Physician Accountability
- Physician cancellation within 48 hours: Full refund + 50% account credit (applied to future appointments/services).
- Physician no‑show (no arrival and no notification within 5 minutes): Full refund + 100% account credit
- Physician late arrival (more than 5 minutes late): Client pays only for time spent, and additional 20% discount applied to any time billed. Client are also offered option to reschedule with same discount applied.
